Attendant Care (ATC)
Attendant Care is designed to maintain an individual’s healthy and safe environment and
assist with their personal care needs. This service is based on specific needs as identified
by the individual (or individual’s family) and assisted by the Direct Support Provider.
Habilitation (HAH)
Habilitation enhances skills in the areas of self-care, home living, communication, social,
health, safety, leisure, self-direction and functional academics. This service is provided
in the members home. This service is based on specific needs as identified by the individual
(or individual’s family) and assisted by the Direct Support Provider.
Homemaker (HSK)
Homemaker service is designed to assist an individual with routine household duties to
ensure the existence of a healthy and safe environment. A Homemaker Agreement is
prepared, and the Direct Support Provider is required to follow the duties outlined in the
Agreement. This service is based on specific needs as identified by the individual
(or individual’s family) and assisted by the Direct Support Provider.
Respite (RSP)
Respite services are designed to provide relief for the primary caregiver. This service provides
for short-term care and supervision. This service is provided in the members home. Respite is
based on specific needs as identified by the individual (or individual’s family) and assisted by the
Direct Support Provider.
